Jaw locked ups in morning even with mouth guard.

I wear a fitted mouth guard from the dentist every night for grinding teeth, so why is it that I am waking up some mornings with my jaw on the left side locked up? I been wearing guard for about 5 months now. I can only open my mouth wide enough for 1 finger. I broke my jaw on the right side a year and a half ago and I sleep on my side. Any advice will be helpful. Thank you..
