Can you explain the MRI results to me and tell me if I have a more muscular or joint issue.

Can you tell me, based on MRI conclusions, what this means...

Left TMJ Early intermediate stage II internal derangement with prominent condylar adaptation and remodeling, and reduced translation.

Right TMJ: Early intermediate stage II internal derangement with joint effusion and restricted translation.

Is there more muscular or joint issues going on?

During the past 3 months I have been attempting 2 x weekly physical therapy, trigger point injections, 2x weekly ultrasounds, TMJ exercises, basic home remedies, and dental evaluation/work. Is there something else I should be considering at this point possibly? So far there has been no relief in my ability to open my mouth, chew, etc. Mouth guard has not been a option because I cannot open my mouth more that 18mm to get fitted.

Can't open mouth

Hello Wendy,

It sounds like you need to see an Oral Surgeon. There are joint effusions in the joint which locks your joint in a closed position.

The surgeon may do an arthocentisis or open TMJ surgery to unlock your joint.

So sorry for the bad news as I rarely recommend this, but see an oral surgeon asap.

Sincerely,
Dr. Spainhower
